Topic: Is your Patient in SHAPE for Discharge?
Description: A brief review of the recent literature on adult hospital readmissionss. This will be followed by a discussion of a quality improvement project undertaken at Methodist Hospital aimed at reducing readmissions.
Audience: Any provider who is involved in the discharge of patients from the hospital.
